Reactivation
Checkbox Enable reactivation:

| Max reactivations | Placeholder -1 for unlimited; hint: Use -1 for unlimited. Leave empty for platform default. |
Notifications section
Empty state: No inline notifications. Add rows to send SMS alerts on activation, usage, or expiration.
